To more effectively combat tuberculosis and other lung diseases in the future, it is essential to gain a deeper understanding of the pathobiology of pathogens, their evolution, modes of transmission, global spread, and interactions with the host. This is the focus of the translational research agenda of the FG MolMyc, which addresses the following key areas: analysis of local and global transmission dynamics, elucidation of resistance and compensatory mechanisms, study of the population structure and evolution of Mtbk strains and other pathogens, description of the virulence, physiology, and pathobiology of mycobacteria, application of high-throughput technologies in research and diagnostics, implementation of new technologies in countries with high incidence, as well as individualized therapy and evolutionary medicine.

 

Methodological equipment

The work of the research group is based on excellent methodological expertise. Next-Generation Sequencing (NGS) methods are used for high-resolution characterization of pathogen genomes and transcriptomes, as well as for analysis of the epigenetic regulation of host genomes. The methodological and technical expertise is based on a modern sequencing platform consisting of Illumina NextSeq 1000 and 2000 systems and a PacBio Vega. In recent years, 10,000 samples per year have been sequenced. In total, genome data from more than 60,000 Mtbk strains and other pathogens are available for further analysis.

A powerful IT infrastructure is available for the bioinformatic analysis of sequence data sets, consisting of a server with over 700 CPU cores, 3.6 TB of RAM, and 1.2 PB of storage space, enabling high-speed processing of large amounts of data.  The bioinformatic analysis methods are continuously adapted to current issues. The in-house developed platforms MTBseq and PhyResSe.org are available for the prediction of resistance-mediating mutations, cluster analyses, and phylogenetic analyses of Mtb genomes.

The pathobiology laboratory has in vitro and in vivo models for investigating clinical Mtbk strains under relevant stress conditions (oxygen deficiency/dormancy, antibiotics, infection) with integrated DNA, RNA, and protein analysis methods. Another focus is on investigating the long-term evolution of Mtbk, including antibiotic selection, hysteresis, tolerance, and persister phenomena in the evolutionary medicine laboratories of safety levels S2 and S3.

Within the research group, a team is dedicated to implementing modern genome-based diagnostics for resistant TB in countries with limited access to diagnostics and infrastructure. It combines expertise in the development and implementation of training concepts, the creation of standardized SOPs, as well as in consulting and troubleshooting, thus enabling sustainable, practical implementation on site in the partner countries.

 

The research group’s agenda has been integrated into a wide range of collaborations and long-term externally funded projects. Notably, this includes involvement in the German Center for Infection Research (DZIF); network projects funded by the German Research Foundation (DFG), such as RTG TransEVO and EXC 2167 PMI; initiatives supported by the Federal Ministry of Research, Technology and Space (BMFTR) (TB-Sequel) and the Federal Ministry of Health (BMG) (a project within the Global Health Protection Programme, GHPP); as well as participation in the EU Innovative Medicines Initiative through the Unite4TB and ERA4TB consortia.

A further milestone for the Molecular and Experimental Mycobacteriology and Diagnostic Mycobacteriology research groups is the reappointment of the FZB as the National Reference Center for Mycobacteria by the Robert Koch Institute (RKI), with the term extending until the end of 2028.

Key projects within the NRZ include the establishment of a nationwide, integrated molecular TB surveillance system in Germany (IMS-TB, in collaboration with the RKI), the continuous advancement of molecular diagnostics in Germany, international collaborations—for example with the WHO—studies on the spread and virulence of mycobacteria, and support for partner laboratories in developing diagnostic capacities, including genome sequencing.

Within DZIF, the research group leads the project “Molecular Epidemiology,” conducting complementary studies on current transmission dynamics and epidemiology across different countries. Longitudinal studies on the spread of MDR-TB strains in Germany (Hamburg, Hannover, Frankfurt) are ongoing, with a particular focus on the interface between TB and migration.

A further key research focus within DZIF, as well as in EXC PMI and international projects, is the development of precision medicine strategies for the treatment of patients with drug-resistant TB (DR-TB). Molecular diagnostics are central to this effort, as phenotypic resistance testing methods are not available for many newer drugs (see figure).

To unravel resistance mechanisms for new antibiotic candidates, long-term evolution experiments are conducted in the Pathobiology Laboratory, where Mtb strains are exposed to antibiotics over extended periods (ERA4TB). This innovative evolutionary approach has proven highly effective in selecting resistant mutants with broad phenotypic and genetic diversity. The resulting data aim to enable the development of diagnostic methods prior to the widespread clinical use of these drugs, ensuring rapid availability and immediate insight into emerging mutations.

These in vitro studies are complemented by large datasets of clinical Mtb strains derived from implementation and surveillance studies. Together, these resources provide ideal conditions to refine genotype–phenotype correlations for predicting drug resistance from genomic data, as well as to analyze the population structure, evolution, and transmission of TB pathogens.
